You do NOT need to write any address.

Dear Sir / Madam

I am writing in connection with a missing or stolen credit card from your bank, which was supposed to have been attached to the letter I received from your bank today. Importantly, I am leaving for a trip in ten days.

Your bank’s letter asked me to acknowledge receipt. However, it was clear the tattered envelope had been interfered with, and importantly, the card was missing, possibly stolen.

I request that you cancel the existing credit card immediately. The card in question is connected to my account number 9876782828 at your Epping branch. I also need you to please arrange to send me a new card, as I am travelling overseas next week.

Finally, could you please advise me within 48 hours by telephone if you are not able to this week either courier the card to my home, or to a bank branch near where I work, as I will then be required to open an account with another bank.  My phone number is:  0444888888.

Yours faithfully,

Smith John
